Many people are eligible for both Medicaid and Medicare, and those that are may be eligible for a Special Needs Plan. Dual-Eligible Special Needs Plans (D-SNPs) are a type of Medicare Advantage plan designed to meet the specific needs of dually eligible beneficiaries. These plans were authorized as part of the Medicare Prescription Drug, Improvement, and Modernization Act of 2003. D-SNP’s combine your Medicare Part A and Part B benefits, Medicare Part D prescription drug coverage, Medicaid benefits and additional health benefits such as vision and dental, into one easy to use plan. Some even include transportation to medical appointments and a healthy food card benefit that gives you funds to spend on healthy foods at approved retailers. Additional benefits will vary by plan, provider, and location.

If you are eligible for both Medicare and financial assistance under Medicaid, you may be eligible for a Dual Eligible Special Needs Plan. If you qualify, you may have a special enrollment period to enroll throughout the year, not only during the Medicare Advantage Annual Enrollment Period (AEP).
